Effective March 1, 2021, the Department of Inspections and Permits will require a Third Party Inspections – Conflict of Interest (COI) form be completed and submitted with all requests for Third Party Certifications for footing, backfill and slab inspections.
The Department resumed acceptance of third party certifications from licensed professionals for these types of inspections February 1, 2021. The Third Party Inspections – Conflict of Interest (COI) form is attached and also available online COI-inspections.pdf
