This is responsible work involving the determination of client needs, linking to appropriate community resources and follow-up to ensure receipt of needed services. The I & A Specialist provides Options Counseling, advocacy and short-term case management work on behalf of seniors, special needs and disabled citizens, and their family members/caregivers. Work involves collaborative and cooperative work with community agencies, helping clients obtain services/housing/other resources, advocating on behalf of clients and families, screening regarding entitlement programs and assisting with application processes via telephone, walk-in contacts, and home visits as needed. Additionally, this position is responsible for staffing outreach events on evenings and/or weekends several times per month. Work may also involve cross-training with the State Health Insurance Program (SHIP) to provide Medicare support during open enrollment periods and as needed throughout the year. Additional duties may be required.

 

Minimum Qualifications:

Graduation from an accredited four-year college or university with a Bachelor’s degree in psychology, counseling, social work, human service, or a related area; knowledge of basic counseling/human services principles; knowledge of social insurance programs; two or more years of experience in working with the elderly and/or adults with disabilities in human service systems.  

NOTE: Graduation from an accredited four-year college or university may be substituted with two (2) or more additional years of experience in working with adults with disabilities or elderly adults in human services systems, for a total of four (4) or more years of experience.

 

Preferences:

Preference given to those who have knowledge, skills and abilities navigating service systems with people with disabilities. Case management experience preferred.
