Case Management and Support Services
The Senior Care Program assists county residents aged 65+ with moderate to severe disabilities who are at risk of institutionalization. Income and asset verification required.
Services Provided *Provided on an as-needed basis
- Case Management
- Home visits every 30-45 days
- Medical Alert Devices*
- Assistance with applying for homeowner’s and renter’s benefits*
- Transportation Assistance*
- Medical Adult Day Care*
- Assistance with personal care* (up to 6 hours per week)
- Meal assistance*
- Gap Filling Funds to purchase medical or other safety equipment*
- Housekeeping*
- Incontinence supplies*
